CPSC, Determined Productions Announce Recall of Plush Toys Sold at Kohl's Department Stores
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission announces the following recall in voluntary cooperation with the firm below. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of product: Plush Frog Stuffed Animal.
Units: 90,000
Manufacturer: Determined Productions Inc., of Larkspur, Calif.
Hazard: The seams in the toy can tear open and expose small plastic pellets, posing a choking or aspiration hazard to young children who mouth the pellets.
Incidents/Injuries: Determined Productions has received one report of seam breakage on the toy, resulting in a child choking on the plastic pellets. The child was taken to the hospital and released without any injury after a procedure to look for aspirated beads.
Description: The recalled toy is an 11 � inch purple stuffed frog with a green chin, belly, hands and feet. The stuffed frog has the word "BRAVE" sewn in purple letters onto the underside of its right foot.
Sold at: Kohl's Department Stores sold the toys exclusively throughout July 2004 for about $5.
Manufactured in: China
Remedy: Consumers are advised to stop using the toy immediately and contact Determined Productions or stop at a local Kohl's Department Stores to receive a refund or store credit.

CPSC, HearthSong Announce Recall of Children's Toy Balls
The following product safety recall was conducted voluntarily by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.
Name of Product: Peek-a-Boo Ball
Units: About 22,500
Manufacturer: HearthSong Inc., of Madison, Va.
Hazard: The wooden rings on the holes of the product may crack, allowing the small wooden heads to fall out and pose a choking hazard to young children.
Incidents/Injuries: The firm has received one report of a child placing the small wooden head in its mouth. No injuries have been reported.
Description: The recalled toy is a blue wooden ball with six holes located equally around the ball. Each hole contains a small wooden head with a smiling face in various colors.
Sold at: Exclusively through HearthSong's catalog and Web site from October 2001 through March 2004 for about $10.
Manufactured In: Thailand
Remedy: HearthSong will notify all known purchasers by mail regarding a refund and a discount towards the next purchase.

CPSC, TOMY Company Announce Recall of Pok�mon Plush Toys
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission announces the following recall in voluntary cooperation with the firm below. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of product: Pok�mon plush dolls, beanbags, and key chains
Units: About 7,400
Retailer: Pok�mon Center NY, of New York, N.Y.
Hazard: Tips of sewing needles have been found in the stuffing posing a puncture hazard.
Incidents/Injuries: None reported.
Description: The recall involves 13 plush Pok�mon characters. They are described below. All recalled toys have a sewn-in label reading "TOMY." There also is a production code on the toy's label that begins with a letter and is followed by two numbers. The following production codes are included in the recall: A04, B04, C04, D04, E03, E04, F03, F04, G03, G04, H03, I03, J03, K03, and L03. Any production code containing an "S" is not part of this recall.
Sold at: Pok�mon Center NY, 10 Rockefeller Plaza, New York City and on the firm's Web site at www.pokemoncenter.com nationwide from January 2004 through August 2004 for between $2 and $11. A limited number were given away as a promotional item.
Manufactured in: China
Remedy: Parents should take these toys away from children immediately and contact TOMY Company for information on receiving a refund or free replacement toy.

#2951 - Charizard Medium Plush � measures about 8-inches tall and is about 9-inches long; it is orange with a yellow stomach, and blue inside its wings.
#2952 - Venusaur Medium Plush � measures about 6-inches tall and 7-inches long; it is turquoise with a pink flower on its back with green leaves.
#2953 - Blastoise Medium Plush � measures about 8-inches tall and is 6�-inches wide; it is light blue with a cream stomach and a brown shell.

#2883 � Kyogre Large Plush � measures about 5-inches tall, 15-inches long, and 21�-inches wide; it is dark blue with red highlights and white accents and a white neck.
#2882 � Groudon Large Plus � measures about 12-inches tall, 16-inches long, and 9-inches wide; it is red with black highlights, white claws, and a gray stomach.
#3087 - Plush Bean Bag Mudkip � measures about 5-inches tall and 6-inches long; it is dark blue with a light blue stomach and tail and orange cheeks.
